Summary: Introduction: Risk management is an integral part of all organizations' processes, and occurs simultaneously with the institution's activities. When referring to health care, Patient Safety is an essential axis of service quality, but the discussion of this topic is still limited in the context of mental health. Thus, seeking to meet a request from the hospital institution in which the master's student works, it was proposed to implement risk management in patient safety in a referral hospital in mental health. This project is aligned with the research line organization and evaluation of health care systems. Objectives: To identify risk factors that influence the safety of patients with mental disorders; develop patient safety protocols; build the diagram of risk management and monitored events in the safety of patients with mental disorders; develop a textual software proposal for internal management of incident notifications. Method: Action research, with a qualitative approach, was used. Participants were the 13 members of the multidisciplinary team responsible for assisting patients with mental disorders. Data collection took place from October 3, 2016 to March 27, 2017. The 13 participants were divided into two groups. Each group participated in 8 meetings, in addition to the closing meeting that was held in a single event with all participants. Results: From the analysis of the textual corpus of the meetings, three categories were obtained: Construction of the protocols; Elaboration of diagram and textual Proposal of software for internal management of incident reports and Research as a collective strategy for the organization of risk management in the safety of patients with mental disorders. Products: Three products were developed: tree diagram of risk management in the safety of patients with mental disorders; Safety Protocols for Patients with Mental Disorders, management technology consisting of seven protocols; textual software proposal for internal management of incident notifications. Conclusion: The management tools were developed with the effective and integral participation of the entire multidisciplinary team. In addition to achieving the objectives, it is noteworthy that there have been some changes in the study institution. The most notable was the creation and official appointment of the Patient Safety Center and the invitation to the researcher to assume the presidency of this service. In this way, this research initiated a process of changing the institutional culture regarding Patient Safety. The products are relevant to scientific and technological development, having the potential to generate impact and be applied at local, regional and national level, as they can be used to operationalize risk management for the safety of patients with mental disorders. The instruments have been incorporated and are in use at the institution.
